By pre-registering by the date noted above you are claiming a t-shirt for your child(ren). We will only be able to guarantee them a shirt, but will do our best to get them as close to thier size as we can. We will have a limited number of shirts and sizes available.

By signing this form I grant permission for Proving Ground Church to take photos/video of my child(ren) during church events and to use these images in church publications, website and social media. I understand that photos and videos will NEVER be accompanied by captions or tags that include names or any other identifying information.

This year we will be serving snacks: Fresh Fruit, Skinny Pop Popcorn, Made Good granola bites, Chocolate covered Strawberry Johnny Pops . If your child may have all of the snacks listed please type ALL, if your child may not have any of the snacks please type NONE. If your child may have some, but not all, please list the snacks your child may NOT have. If you comment anything besides ALL you will be contacted by Tiffany. Please email Tiffany@provinggroundchurch.com for additional information regarding the handling of snacks, cross contamination and snack alternatives. You may also provide your own snacks each night, that can be handed to a volunteer at check in.
