By pre-registering you are claiming a t-shirt for your child(ren). We will only be able to guarantee them a shirt, but will do our best to get them as close to thier size as we can. We will have a limited number of shirts and sizes available.

By signing this form I grant permission for Proving Ground Church to take photos/video of my child(ren) during church events and to use these images in church publications, website and social media. I understand that photos and videos will NEVER be accompanied by captions or tags that include names or any other identifying information.

VBS 2026 Surprise Party Liability release By signing you agree to the following statement. I give permission for my child to participate in VBS at Proving Ground Church of Lakehurst, NJ. As parent/legal guardian of said minor, I accept full responsibility for any medical costs incurred in the event of an accident or other emergency.* After reasonable efforts have been made to contact me or my spouse I grant the volunteers of PGC permission to make any and all necessary medical decisions for my child until I arrive

Your signature indicates that you agree to the following statement and are granting the said minor (Grades entering 1st-entering 6th) above permission to participate in a foam party. Your minor will be given safety goggles and a shirt to enter the foam. They will have the option to remove their shoes. Baby wipes will be provided to clean themselves up afterwards. Proving Ground Church is requiring all participants to bring a change of clothing and towel on the evening of this event (a reminder will be provided). Proving Ground Church is not, as mentioned in the liability waiver, responsible for any medical costs incurred in the event of an accident or other emergency that may take place during this activity, furthermore, Proving Ground Church is not responsible for any damaged personal property that may be a result of the foam party

NONE if your child may not have any of the snacks listed or SOME followed by the snacks they ARE NOT permitted to eat from the list: Miss Jones Natural Vanilla frosting, Miss Jones Natural Chocolate frosting, Miss Jones Gluten-free classic yellow cake mix (PLEASE NOTE: prepared with milk, eggs and butter), Unreal Milk Chocolate Gems, Enjoy Life Chocolate Chips, Bettergoods dye free rainbow sprinkles, Fresh Fruit: strawberries and watermelon, Hershey's Simply 5 Chocolate Syrup, Skinny Pop Popcorn.
